Management Of Haemmoroids through Ayurveda
This disease is characterized by fleshy sprouts in anorectal region and agonizing pain to the patients. The treatment is generally, preventive, symptomatic and palliative in modern medicine. Surgery is a last resort. In addition to the medical and surgical management, certain Ayurvedic para-surgical procedures, namely, Ksarakarma, Ksarasutra, Raktavasecana & Agnikarma are also recommended and practiced.
Clinical study
i) The study to evaluate the six-fold approach to the treatment, has been taken up in the Clinical Research Enquiry of the CCRAS at BHU, Varanasi to evaluate Ksara sutra in comparison to plain thread ligation. The outcome of the study conducted on 800 cases of haemorrhoids and 200 cases of centinal piles and anal warts recommended and practiced.
* Medicinal treatment is a therapy that can be advised in haemorrhoidal patients having milder or vague complaints with satisfactory results.
* Ksarakarma or chemical cautery is an effective measure in control of bleedings of internal haemorrhoids with small ill defined harmorrhoidal mass.
* Ksara sutra ligation is a therapy of choice in severe bleeding cases having well pedunculated internal hemorrhoids. It has added advantages of chemical action in the surrounding tissue over modern conventional ligation therapy measures. The results are so good as surgical ligation and excision.
* In multiple haemorrhoids or externo- internal haemorrhoids, surgery is a therapy to be advised and simple ligation and excision technique should be preferred because of its equally good results and minor, easily manageable complications.
* Raktavasecana or bloodletting is a palliative therapy helpful in relieving severe pain, protrusion and oedema of thrombosed haemorrhoids.
* Agnikarma in the form of electrical thermal cautery is a suitable procedure in the excision of sentinel piles and anal warts falling in Vataja type of Arsa.
* The internal medical treatment as an associated complementary therapy has a very important bearing on the control of precipitation of disease, incidence or recurrences and complications.
ii) Further studies have been conducted with two regimens:
Ksara sutra application and internal medication with Kankayan Vati and Triphala Churna and local application of Kashishadi Taila on 774 patients. The observations indicate positive response to the treatment ranging from 85-95% with a less recurrence rate.
